TPH is hosting a School Mobile Clinic, on June 11, 2022 from 12:00PM – 6:00PM at Lawrence Park Collegiate Institute.
– School-based vaccines (Meningococcal ACYW135, Hepatitis B and HPV vaccines) for eligible individuals in Grade 7-12
– Dental and oral health screening
– COVID-19 vaccine for individuals ages 5+
Kindly Note:
– Walk-ins are welcome however appointments are highly encouraged
– For school-based vaccines – please bring consent form(s), a copy of student’s immunization record and health card, if available, or other identification
– For COVID-19 vaccines – please bring consent form, OHIP not required but please bring some form of identification
– Masks are required at the School Mobile Clinic
